DescriptionClinical Program Coordinator RN - Swedish Aortic CenterPart TimeDay ShiftThe Clinical Program Coordinator is responsible for the planning and coordination of the Swedish Aortic Center (SAC) within the Swedish Heart & Vascular Institute (SHVI) throughout the entire patient experience including patient referral intake, follow-up coordination, patient care progression facilitation and tracking, patient database management, and quality and operational metrics tracking. The coordinator role is designed to optimize patient, staff and physician integration within the SAC program at Swedish, a joint program between Cardiovascular Surgery and Vascular Surgery specialties. They will also serve as the Liaison with other programs within Swedish, Providence and other local/regional hospitals. The Coordinator is also responsible for providing support in the development, implementation of other programs as they relate to SAC at Swedish & SHVI, such as Cardiology, Primary Care, Emergency and Acute Care, but not limited to these programs.Providence caregivers are not simply valued - they’re invaluable.
