SUMMARY

This position is primarily responsible for managing program services for enrolled participants including the development of care plans, provision of linkages and referrals to care and follow-up activities, communication with medical/behavioral health providers and project staff to decrease barriers to access for medical care and support services. Assists in outreach and health education activities, as needed. Recruits individuals for, and conducts, evidence-based prevention interventions and counseling. Conducts data entry. This position reports to the Program Coordinator.

Education and/or Experience:

Bachelor’s Degree and CASAC required; at least 3 years of experience in HIV/AIDs or behavioral health; Certified in HIV/Viral Hepatitis Testing and Pre/Post Testing Counseling required; bilingual in English/Spanish preferred.High School Diploma or GED – CASAC-T with 4 years’ experience working with applicable populations who have substance abuse/co-occurring disorders.Good verbal and written communications skillsKnowledge of HIV/AIDS, substance use & mental illness, and LGBT issuesGood problem-solving skills and ability to work autonomouslyMust have computer skills (Microsoft Office, etc.)Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.
